Our Registered Nurses can undertake a health assessment to help determine your care needs. This can be done as part of a Government funded package or privately.
Once completed, the health assessment can be used to help to decide what services are required to maintain a client’s independence and ensure they are able to continue to live at home with the help needed.
02 / CARE PLANNING
Care Planning, Case Management & Community Resource Coordination
Once an assessment has been completed, your case manager will develop a care plan for the client and, when required, their family.
This will detail the services that have been identified and the days and times that are preferred.
Services may include personal care, cleaning or gardening services, meal provision, or medication administration.
Your case manager is the key liaison point, ensuring continuity of care and familiarity for our clients and their families.
03 / NURSING SERVICES
General Nursing Services
Rural Remote Home Care Services nursing staff provides a full range of nursing services in the home, ranging from showering assistance to medication management.
Our nurses are also able to provide advice and education on appropriate safety equipment for the home and refer to an Occupational Therapist for specialist equipment if it is identified that this is required.
They will liaise with the doctor and assist with the coordination of medical and other allied health professional appointments. We can advise on products and support for you in the following areas:
Palliative Care is specialised, multi-disciplinary and holistic care for someone suffering from an advanced, progressive terminal illness.
Rural Remote Home Care Services nursing staff and support workers provide health care support, medication administration, comfort and advice to palliative care patients and their families.
We work closely with Palliative Care Teams and Community Nursing Teams in a multi-disciplinary approach to your care.
05 / EDUCATION
Education for Clients and Carers
We provide information and support to the patient and their support system.
This may include lifestyle maintenance, nutritional advice and practical “Preparation for the Day” planning.
